Claim Instructions

  • All claims must be approved by prior to work being completed
  • For all contracts purchased prior to May 1, 2008 please have your repair shop call 800-745-4441
  • For all contracts purchased after May 1, 2008 please have your repair shop call 800-476-0770

Cancellation Policy

  • All cancellations request take 30 days to process. A refund will be sent to you directly.
  • If you have a question please contact 800-323-4300 option #2

For all contracts purchased prior to May 1, 2008 (Administrator: Phoenix American)

  • Write a letter requesting to cancel and sign it - stating why you want to cancel(must have their current address)
  • Include a copy of the declaration page/application you recieved from Pheonix American (must indicate on letter if document is lost)
  • If it was traded in, please include a copy of the Odometer Statement
  • If vechicle was sold as personal sale, must include date of sale in letter and mileage as of date of sale
  • And if you would like to cancel for any other reason, the letter must include mileage as of date of letter
